    摘要:

    磨损产生的聚乙烯颗粒会引发炎症反应、骨溶解、无菌松动等一系列问题,最终导致人工关节假体植入失败。介绍现行髋关节模拟试验标准以及常见的髋关节模拟试验机设备,通过对髋关节模拟试验研究CoCrMo/UHMWPE型人工髋关节假体磨损性能的相关文献的综述,从球头直径、髋臼外展角、球头表面粗糙度、载荷、蛋白浓度和材料改性6个方面讨论髋关节假体磨损性能影响的研究进展。其中关于球头直径、球头表面粗糙度以及载荷3个参数对髋关节假体磨损性能的影响规律的观点是一致的,即3个参数与髋关节假体磨损性能成反比,而关于髋臼外展角与蛋白浓度对髋关节假体磨损性能影响规律的观点还存在分歧,需进一步探索。展望假体设计的研究方向与试验参数的优化方向,以期为新一代髋关节假体的优化设计及临床研究提供一定的参考和指导。

    Abstract:

    Polyethylene particles produced by wear can cause a series of problems,such as inflammation,osteolysis,aseptic loosening,etc.,and eventually lead to the failure of artificial joint prosthesis implantation.The current hip joint simulation test standards and common hip joint simulators were introduced,the relevant literatures on the wear performance of CoCrMo/UHMWPE artificial hip prosthesis through hip joint simulation test were reviewed,and the influence on the wear performance of hip joint prosthesis was discussed from six aspects,including ball head diameter,acetabular abduction angle,ball surface roughness,load,protein concentration and material modification.There are consistent views on the influence of ball head diameter,ball head surface roughness and load on the wear performance of hip joint prosthesis,that is,the three parameters are inversely proportional to the wear performance of hip joint prosthesis.While there are still divergent views on the influence of acetabular abduction angle and protein concentration on the wear performance of hip joint prosthesis,which needs to be further explored.The research direction of the prosthesis design and the optimization direction of the test parameters were prospected,which provides some reference and guidance for the optimization design and clinical research of the new generation of hip prosthesis.
